I am an associate professor in the Department of Psychiatry at the University of Cambridge and an honorary consultant liaison psychiatrist in Cambridgeshire & Peterborough NHS Foundation Trust (CPFT) and Cambridge University Hospitals NHS Foundation Trust (CUH). My background is in behavioural/cognitive neuroscience (BA 1996, PhD 2001, MD 2006), general medicine (MB BChir 2001, MRCP 2007, FRCP 2023), and general adult/liaison psychiatry (MRCPsych 2009, CCT 2015, FRCPsych 2021). My work spans liaison psychiatry, computational psychiatry, and clinical informatics. Psychiatric disorders are common, debilitating, and sometimes associated with substantial loss of health and lifespan. Our research group applies computers to psychiatry in several ways, trying to improve our understanding and treatment of mental health problems. First, we use computer models to help understand how the mind works and how it sometimes goes wrong. This involves building and testing mathematical models of how psychological processes drive behaviour, and how they go awry. We collaborate with other groups across Cambridge and the world, ranging from labs conducting sophisticated behavioural neuroscience to those testing cognition in human volunteers with psychiatric disorders. For example, we have shown how decreases in the levels of dopamine in the orbitofrontal cortex may be involved in some of the early stages of schizophrenia, how dopamine is involved in learning from losses, how the prefrontal cortex helps to regulate negative emotion, and how cognition is altered in addictions or obsessive–compulsive disorder. Second, we help researchers and health services collect better data and use it for clinical care and for research. We create free secure software allowing patients to collect both simple and advanced measures of their mental state and brain function, and provide it to health services such as the UK National Health Service (NHS). Our hope is that initiatives like this will steadily improve care. We create other free software that can de-identify clinical records for research at a large scale, can support recruitment to research studies being conducted in collaboration with the NHS, and can help to link data across organizations safely and securely. We have conducted research nationally into the public’s preference for how their data should be used and shared, for their own care and for research to benefit others. We help to lead digital innovation in the NHS locally. Third, we use NHS data to try to discover new things of clinical importance. For example, during COVID-19 we published the world’s first study showing that people with severe mental illnesses were at disproportionate overall risk of death. We have contributed to the evidence that lithium might be protective against some kinds of dementia. And we have shown that simple measures like birth weight can predict the risk of someone with schizophrenia developing diabetes when on some kinds of medication. Clinically, I am a consultant liaison psychiatrist working with people aged 17–64. Liaison psychiatrists work in general hospitals, in my case Addenbrooke’s Hospital and the Rosie Hospital in Cambridge. These hospitals provide acute general and maternity services, plus specialist services including transplantation, tertiary neurology/neurosurgery services, and the major trauma service for the East of England. The mind arises from the brain, and mind and body are intimately related, and we teach current and future practitioners about these relationships.
